Information materials
on the documents concerning those who are eligible for off-competition, first-priority, targeted destinations and for those who have achieved significant success in the study of specialized subjects

The applicant to the institution of higher education shall personally present the document of the state sample on the previously obtained educational (qualification) level, on the basis of which the application is made and the supplement thereto; Certificate(s) of the Ukrainian Center for Educational Quality Assessment (for applicants based on a complete general secondary education); medical certificate in the form 086-о; passport of a citizen of Ukraine (passport of a citizen of Ukraine for traveling abroad, a military ticket or a certificate of birth, birth certificate – for those who do not have a passport due to age, or another document certifying the person and their citizenship) and documents confirming the right to apply without competition, to the first priority enrollment, by interview, by targeted directions and documents confirming the achievement of outstanding success in the study of profile subjects. All copies of the documents shall be certified by the originals of the higher education institution to which they are submitted or in accordance with the procedure established by law.

Categories of individuals eligible to enter out of competition in accordance with paragraph 13.1 of the Conditions of admission to higher education institutions of Ukraine

  І. Individuals to whom the Law of Ukraine “On the Status of War Veterans, Guarantees of their Social Protection” is granted such right

For a combatant
(Article 12).

  • Proof of combatant actions.

 

 

For underage children of combatants killed as a result of wounds, or injuries sustained during the war (Article 15).

  • Birth certificate of the enrollee, death certificate of the combatant;
  • Certificate of identification from a military enlistment office on the basis of the death of a parent due to military activities in the territories of other countries with a signed, affixed seal;
  • Certificate of the child (mother) as a member of the family of a fallen (deceased) war veteran.

 

 

 

ІІ. Orphaned children deprived of parental care, aged 18 to 23 years old (Decree of the Cabinet of Ministers of Ukraine from April 5, 1994 №226 “On improving the education, teaching, social protection and material support of orphans and children left without parental care”

An orphaned child whose parents died or were killed.

  • Birth certificate of the applicant;
  • A copy of the document on the decision of the Children's Service to grant the status of orphan or child deprived of parental care in the district of the Kyiv and Sevastopol state administration, the executive body of the city or district in the city council at the place of the child’s origin.

 

Children deprived of parental care due to:
a) the deprivation of parental rights of the parents;

  • Birth certificate of the applicant;
  • A copy of the document on the decision of the Children's Service to grant the status of orphan or child deprived of parental care in the district of the Kyiv and Sevastopol state administration, the executive body of the city or district in the city council at the place of the child’s origin.

 

b) Parental deprivation without the termination of parental rights;

  • Birth certificate of the applicant;
  • A copy of the document on the decision of the Children's Service to grant the status of orphan or child deprived of parental care in the district of the Kyiv and Sevastopol state administration, the executive body of the city or district in the city council at the place of the child’s origin..

 

c) Recognition of parents missing or incapacitated, declaring parents dead;

  • Birth certificate of the applicant;
  • A copy of the document on the decision of the Children's Service to grant the status of orphan or child deprived of parental care in the district of the Kyiv and Sevastopol state administration, the executive body of the city or district in the city council at the place of the child’s origin.

 

d) Parents serving a sentence of imprisonment or being held in custody at the time of the investigation;

  • Birth certificate of the applicant;
  • A copy of the document on the decision of the Children's Service to grant the status of orphan or child deprived of parental care in the district of the Kyiv and Sevastopol state administration, the executive body of the city or district in the city council at the place of the child’s origin.

 

e) The search for the parents by law enforcement agencies related to child support evasion and lack of location information;

  • Birth certificate of the applicant;
  • A copy of the document on the decision of the Children's Service to grant the status of orphan or child deprived of parental care in the district of the Kyiv and Sevastopol state administration, the executive body of the city or district in the city council at the place of the child’s origin.

 

f) Prolonged parental illness that prevents them from fulfilling their parenting responsibilities;

  • Birth certificate of the applicant;
  • A copy of the document on the decision of the Children's Service to grant the status of orphan or child deprived of parental care in the district of the Kyiv and Sevastopol state administration, the executive body of the city or district in the city council at the place of the child’s origin.

 

g) Abandoned children whose parents are unknown; children abandoned by parents; and street children.

  • Birth certificate of the applicant;
  • A copy of the document on the decision of the Children's Service to grant the status of orphan or child deprived of parental care in the district of the Kyiv and Sevastopol state administration, the executive body of the city or district in the city council at the place of the child’s origin.

 

Individuals from 18 to 23 years old from orphans and children deprived of parental care.

  • Birth certificate of the applicant;
  • A copy of the document on the decision of the Children's Service to grant the status of orphan or child deprived of parental care in the district of the Kyiv and Sevastopol state administration, the executive body of the city or district in the city council at the place of the child’s origin.

 

At present, the issue of granting the status of orphan or child deprived of parental care is regulated by the Procedure of conducting done by authorities regarding guardianship and custody related to the protection of the child’s rights, approved by the Cabinet of Ministers of Ukraine from 24.09.2008 № 866 (hereinafter - the Procedure) which came into effect 17.10.2008.
According to paragraph 22 of the Order of the District, the city administration of Kyiv and Sevastopol, the executive body of the city or district in the city council of the child’s place of origin, upon the submission of the children's affairs service, decides to grant the status of orphaned child or child deprived of parental care. The said decision is an official document confirming the status of orphan or child deprived of parental care.
The decision shall state the surname, first name and patronymic of the child, date of birth, the circumstances under which he or she was left without parental care, documents supporting these circumstances, and the form of the arrangement of the child.
For children who have been orphaned before October 17, 2008 and for whom no separate decision on granting status has been made, an extract from the orphan and the child deprived of parental care card printed from a single electronic data bank may serve as an extract and certified in due course by the head of the guardianship authority.
Hereafter, if the person's entitlement to the benefit depends on their attaining a certain age (in particular, 18 or 23 years), they may use it if they have not attained that age at the time of applying for a higher education. Even if the documents are submitted on the day of reaching the specified age, the person still has the privilege. Individuals who submit documents the day after they reach the age are not entitled to the privilege.

 

ІІІ. Disabled people of group I and II and disabled children who are not contraindicated to study in the chosen field (specialty) (Law of Ukraine “On the basics of social protection of disabled people in Ukraine”)

Disabled I and II groups who are not contraindicated in their chosen specialty.

  • A certificate (of a pension or recipient of state social assistance in which the disability group is indicated)

 

Children with disabilities

  • A certificate (pension or beneficiary of state social assistance).

If the certificate is issued to one of the parents, it must clearly state the last name and patronymic of the disabled child as well as the year of birth.

 

IV. Individuals who have been granted such a right by the Law of Ukraine “On the status and social protection of citizens affected by the Chernobyl disaster”
 (documents giving entitlement to benefits should be issued or re-registered after 06.06.1996)

Category 1
(point 20 article 26).

  • A blue certificate, Series A (see example in Annex 2).

 

 

Category 2
(point 26 article 20 through point 1 article 21).

  • A gray certificate, Series B, a light-green certificate Series B (see example in Annex 2).

 

Category 3
(point 6 article 22).

  • A green certificate, Series B (see example in Annex 2);
  • A certificate of permanent residence in the territory of radioactive contamination, confirmed by registration in the passport, destination, copy of the contract.

 

Subject to entry out of competition in accordance with paragraph 6 of Article 22 of the Law of Ukraine "On the status and social protection of citizens affected by the Chernobyl disaster" is verified with a stamp, signed by the Minister of Education and Science of the Autonomous Republic of Crimea, the heads of  the main departments of the State Land Committee, state departments of environmental protection, the departments of forestry and hunting, the main departments of education and science of the oblast, the Kyiv Sevastopol city state administrations.
A certified copy of the agreement between the higher education institution and the Ministry of Education and Science of the Autonomous Republic of Crimea, the main departments of education and science of regional, Kyiv and Sevastopol city state administrations.

Category 4
(point 6 article 22 through point 1
article 23).

  • A brown certificate, Series B (see example in Annex 2);
  • A certificate of permanent residence in the territory of radioactive contamination, confirmed by registration in the passport, destination, copy of the contract.

Subject to entry out of competition in accordance with paragraph 6 of Article 22 of the Law of Ukraine "On the status and social protection of citizens affected by the Chernobyl disaster" is verified with a stamp, signed by the Minister of Education and Science of the Autonomous Republic of Crimea, the heads of  the main departments of the State Land Committee, state departments of environmental protection, the departments of forestry and hunting, the main departments of education and science of the oblast, the Kyiv Sevastopol city state administrations.
A certified copy of the agreement between the higher education institution and the Ministry of Education and Science of the Autonomous Republic of Crimea, the main departments of education and science of regional, Kyiv and Sevastopol city state administrations.

Children under the age of 18 that are disabled due to the Chernobyl disaster
(point 4 part 3 article 30).

  • A yellow certificate, Series D and a Child Certificate Card, affixed with the seal of the Regional State Administration, the City State Administration of Kyiv and Sevastopol (see example in Annex 2).

 

 

Children under the age of 18 whose parents have died in regards to category 1 or 2 and whose death is related to the Chernobyl disaster
(point 26 article 20 through part 2 article 20 and part 2 article 21).

  • Birth certificate of the applicant;
  • Death certificate of one (both) parents;
  • Expert opinion on the death of one (both) parents who have been linked to the Chernobyl disaster and a parental category certificate (see example in Annex 2). 

The expert opinion of the Interagency Expert Panel on the Causation of Diseases, Disability and Death by Ionizing Radiation and Other Harmful Factors as a result of the Chernobyl Nuclear Power Plant Accident or Military Medical Commissions in the Ministry of Internal Affairs and the Ministry of Defense.

Children under the age of 18 whose parents have died in regards to category 3 as a Chernobyl disaster participant and whose death is related to the disaster
(point 26 article 20 through part 2 article 22).

  • Birth certificate of the applicant;
  • Death certificate of one (both) parents;
  • Expert opinion on the death of one (both) parents who have been linked to the Chernobyl disaster and a parental category certificate (see example in Annex 2).

 

The expert opinion of the Interagency Expert Panel on the Causation of Diseases, Disability and Death by Ionizing Radiation and Other Harmful Factors as a result of the Chernobyl Nuclear Power Plant Accident or Military Medical Commissions in the Ministry of Internal Affairs and the Ministry of Defense.

Children under the age of 18 whose parents are categorized as 1
(part 2 article 20 through point 26 article 20).

  • Birth certificate of the applicant and the certificate of category 1 of the father (mother) with tab of the established sample;

Certificate supplement (see sample in Annex 2).

 

 

V. Individuals who have been granted such a right by the Law of Ukraine “On enhancing the prestige of mining work”

Miners with an underground work experience of at least three years.

  • Certificate of the personnel department of the enterprise (mine management), which stores the employment records of workers performing mining work, referred to in Article 1 of the Law of Ukraine "On enhancing the prestige of mining work" or a certificate of management of the Pension Fund of Ukraine, where the employee shall be retired, one who performed mining and mining operations referred to in Article 1 of the Law of Ukraine "On enhancing the prestige of mining work", signed by the Head of Department secured with the appropriate seal;
  • Copy or extract from the work book with the signatures affixed by the seal of the HR department.

Certificate signed by the head of the department and the head of the enterprise (mine management) and certified by the appropriate seal of the legal entity with its identification code.

Children whose parents have at least fifteen years of underground work experience.

  • Birth certificate of the applicant;
  • Certificate of the department of personnel of the enterprise (mine management), which stores the employment records of workers performingmining work, referred to in Article 1 of the Law or certificate of management of the Pension Fund of Ukraine, where the employee shall be retired, one who performed mining and mining operations referred to in Article 1 of Law No. 345, signed by the Head of Department secured with the appropriate seal;
  • Copy or extract from the work book with the signatures affixed by the seal of the HR department.

Certificate signed by the head of the personnel department and the head of the enterprise (mine management), affixed with the appropriate seal of the legal entity, indicating its identification code.

Children of miners killed as a result of an industrial accident.

  • Birth certificate of the applicant and the death certificate of one or both parents and one of the following documents:
  • Certificate of a family member of a dead miner;
  • Occupational accident report (Form H-1).

 

Children of miners with disabilities of groups I and II.

  • Birth certificate of the applicant and pension certificate of the father (mother) indicating the group and cause of disability or certificate of recipient of state social assistance indicating the group and cause of disability and one of the following documents:
  • Opinion of the Center for medical-social examination on the existence of a causal link to disability with work injury.
  • Industrial Accident Act (Form H-1).

The conclusion of the Center for medical-social examination must be signed by the Chairman of the Center for medical-social examination (the signature is deciphered and sealed).

 

VІ. Children whose parents died or became disabled at coal mines when entering mining specialties (Decree of the President of Ukraine “On State Aid to Children Studying in Mining Specialties and whose Parents were Killed or Disabled at Coal Mines” from 19.05.1999 № 524)

Children whose parents were killed in coal mines when entering mining majors.

  • Birth certificate of the applicant and death certificate of one of the parents and one of the following documents:
  • Certificate confirming the status of the family member of the dead miner and his entitlement to benefits;
  • A supplement outlining the rights and social guarantees of a family member of a dead miner, as provided by law.

 

 

Children whose parents became disabled at coal mines when entering mining majors.

  • Birth certificate of the applicant and the certificate (pension or beneficiary of state social assistance, indicating the group and the reason for the disability);
  • A Center for medical-social examination certificate, linking the disability to work injury.

 

  • The certificate of the Center for medical-social examination must be signed by the Chairman of the Center for medical-social examination(the signature is deciphered and sealed).

VІІ. Children of servicemen of the Armed Forces of Ukraine, other military formations, law enforcement officers who were killed in the course of their official duties, Decree of the President of Ukraine "On additional measures to strengthen the care of the defenders of the Fatherland, their legal and social protection, improvement of military-patriotic education of the youth"
from 21.02.2002 № 217)

Children of military personnel of the Armed Forces of Ukraine, other military formations, law enforcement officers who were killed in the line of duty.

  • Birth certificate of the applicant and death certificate of one of the parents and one of the following documents:
  • Family ID of the deceased serviceman;
  • Excerpt from the order of the military unit on the fact of the death of a serviceman or a certificate from the military registration and enlistment office, a certificate from the law enforcement agency where the law enforcement officer served, with signatures, affixed seals, about the death in connection with the performance of official duties.

 

 

VІІІ. Members of the families of miners and rescuers who died as a result of an accident at the rental company mine named after A. F. Zasyadko, according to Annex 6 to the Cabinet of Ministers of Ukraine from January 9, 2008 № 6 “Some Issues of Social Protection for Members of the Families of Miners and rescuers”

Members of the families of miners and rescuers who died as a result of an accident at the rental mine named after A. F. Zasyadko.

  • Birth certificate of the applicant and death certificate of one of the parents, as well as one of the following documents:
  • Certificate confirming the status of the family member of the dead miner and his / her right to appropriate benefits and compensation;
  • A supplement outlining the rights and social guarantees of a family member of a dead miner, as provided by law.

 

Categories of individuals who, according to clauses 8.1, 8.2, 8.3, 8.4 of the Conditions of admission to higher educational establishments of Ukraine, are entitled to participate in the competition based on the results of entrance examinations in competitive subjects

  • Birth certificate of the applicant;
  • Pension or state social assistance beneficiary's certificate.

If the certificate is issued to one of the parents, it must clearly state the name of the disabled child, affixed to the seal of the issuing authority.

Servicemen of the Armed Forces of Ukraine, other military units formed in accordance with the Laws of Ukraine, as well as law enforcement agencies of the special purpose and the State Special Transport Service, who serve military service.

  • Military ID.

The military ID must indicate the fact of military service.

Citizens of Ukraine dismissed from conscription in the year of entry into higher education

  • Military ID.

The military ID must indicate the fact of enrollment or dismissal from military service in the year of admission to higher education.

Servicemen of the sergeant rank who are under contract military service (upon entering the correspondence form).

  • Military ID.

The military ID must indicate the fact of military service.

Individuals with diseases listed in the list of diseases that may be an obstacle to citizens passing an external independent evaluation established by the Ministry of Education and Science of Ukraine and the Ministry of Health of Ukraine, for which the Ukrainian Center for Educational Quality Assessment failed to ensure the completion of an independent external evaluation with educational achievements.

  • Certificate of illness according to the list.

The certificate must be issued in the manner specified by the Ministry of Health dated 24.04.2009 № 04.04.44-8-325. The certificate of the medical-preventive institution is issued at the place of residence of the applicant, and is issued by the medical-control commission of the institution, taking into account the List of diseases that may be an obstacle to the external independent evaluation, approved by the order of the Ministry of Education and Science of Ukraine dated 25.02.2008 № 124/95.
The certificate is issued at the request of the applicant, the parents of the applicant or the individuals who replace them, in a free form signed by the head of the medical-preventive institution and certified by a round seal of the institution.

Individuals who have completed the general secondary education in 2007 and earlier (at the enrollment to both full-time and part-time forms of study)

  • Complete Secondary Education Certificate, issued in 2007 and earlier.

 

Citizens of Ukraine who, in the year of entry, have completed a comprehensive secondary education abroad.

  • Document for the completion of a comprehensive secondary education in 2011 abroad.

 

Categories of individuals entering higher education institutions by purpose of admission in accordance with paragraph 10.1 of the Conditions of admission to higher education institutions of Ukraine

  • Document of the state sample about the complete general secondary education with honors or with excellent grades (10, 11, 12 points).

 

The certificate of assignment after the age of 18 must be issued by the regional state administration with a signature affixed with the seal.

Children under the age of 18 whose parent has died in category 1 or 2 whose death is related to the Chernobyl disaster
(point 26 article 20 through part 2 article 20 and part 2 article 21).

  • Birth certificate of the applicant;
  • Death certificate of one (both) parents;
  • Death certificate of one (both) parents who have a connection with the Chernobyl disaster and certification of the category of the parents (see example in Annex 2);
  • Document of the state sample about the complete general secondary education with honors or with excellent grades (10, 11, 12 points).

 

The certificate must be issued by the district or regional state administrations signed with the seal affixed.

Children under the age of 18 whose parent has died in category 3 whose death is related to the Chernobyl disaster
(point 26 article 20 through part 2 article 22).

  • Birth certificate of the applicant;
  • Death certificate of one (both) parents;
  • Death certificate of one (both) parents who have a connection with the Chernobyl disaster and certification of the category of the parents (see example in Annex 2);
  • Document of the state sample about the complete general secondary education with honors or with excellent grades (10, 11, 12 points).

 

The certificate must be issued by the district or regional state administrations signed with the seal affixed.

Children under the age of 18 whose parents are categorized as 1
(part 2 article 20 through point 26 article 20).

  • Birth certificate of the applicant and certification of the category of the parents;
  • Supplement to the certification (see sample in Annex 2);
  • Document of the state sample about the complete general secondary education with honors or with excellent grades (10, 11, 12 points).

 

 

Categories of individuals who have achieved significant success in the study of specialized disciplines and are eligible for enrollment outside the competition in accordance with clause 12.1 of the Conditions of admission to higher education institutions of Ukraine
